Synovial Cysts Cause Back and Leg Pain, Arctic Chiropractic, Sitka Offers Relief

SYNOVIAL CYST FACTS

First, let’s discuss what synovial cysts are. Synovial cysts are uncommon lesions associated with facet degeneration and usually found in the lumbar spine at L4-S1. (1) 33% to 40% of synovial cysts are linked with degenerative spondylolisthesis and instability of the affected joint. (2,3) Synovial cysts may cause sciatica/leg pain. (4) Synovial cysts appear in females mostly in their sixth decade of life, giving rise to radicular pain and neurological deficits. These cysts are frequently seen at L4-5 which is also the site of maximum mobility in the lumbar spine. MRI is the diagnostic tool of choice as it shows the texture of the cyst (more or less fluid). (5) When synovial cysts appear lighter on MRI T2 images, they are probably more fluid filled and less probably gelatinous or calcified, making them potentially easier to ease. (6) Synovial cysts have been documented to disappear particularly if the cyst wall is not calcified. SYNOVIAL CYST CAUSES

Wilby et al describe a channel that joins the facet joint capsule with the ligamentum flavum, the area where the synovial cysts become visible. At surgery for these cysts, they discovered that fragments of articular cartilage and bone were in the walls of the channels in 89% of the cysts. This debris jams and prevents the channel from draining and provokes granulation tissue and scar formation probably leading to synovial cyst formation.
